registration (pdf) form when registering for the Congress and social
programme. Confirmation will be sent upon receipt of payment. In order
to receive an e-mail confirmation, please indicate your e-mail address
on the registration form. For payment details, please see below. Registration
for events that are included in the registration fee will only be confirmed
if ticked ”yes” on the registration form. Please note that registration
for the various tours and events will be confirmed upon payment on a ”first

German tax legislation requires participants to pay their registration fee either including or excluding German VAT. Therefore, please identify your category below and choose the appropriate alternative on the registration form. Including 16% VAT: Registration fees paid by non-taxable entities (institutions/organisations etc) and private persons. All registration fees for accompanying persons. THE REGISTRATION FEE FOR ACCOMPANYING PERSONS INCLUDES: Public Transportation Card, Munich Orientation Tour, 26 June, EUR 50 discount on one Social Tour of your choice, Opening Ceremony and Welcome Reception, 26 June, Congress Evening/All-Congress Event, 27 June. Please note that the registered accompanying person’s fee does not include admission to the scientific sessions or the Congress documentation. STUDENT REGISTRATION PhD students, MD students or fellows in training, under the age of 35 are eligible for the student fee. Student status must be certified by a Head of Department and a copy of an ID card or passport must be submitted with the registration. MEET THE EXPERTS EUR 30 per seminar, including lunch. Meet the Experts sessions will be available during lunch time all five days of the Congress. More information will be provided in the final programme and registration will be possible on site. NAME CHANGE Should you be prevented from attending, you may be substituted by a colleague. An administrative fee of EUR 50 will then be charged. EAACI MEMBERS Please note that it is necessary to have settled your membership fee in order to benefit from the reduction on the registration fee. EAACI Junior/Affiliate Junior Members are eligible for the student registration fee. Before the start of the Congress, appointments for pre pick up of the Congress material can be made for preregistered groups. Please contact Interplan AG for further information, email: wac2005@interplan.de CANCELLATION OF REGISTRATION Notification of cancellation must be sent in writing to Congrex. Cancellations of registrations will be accepted until 10 May 2005, up to which date the total amount will be refunded less EUR 75 for administrative expenses.We regret that refunds cannot be made for cancellations received after 10 May 2005.
